Oracle Fusion Learning Cloud: Learning Incident Approval Process Is Failing

When a user submits a Learning Incident, the approval rules configured in the Transaction Console are failing.
This occurs when the user first submits the Learning Incident report before it reaches any approvers.
Steps to Reproduce:
- Go to Me > Learning > Find a Course
- Click Actions > Report > Inappropriate Content > Submit
- Approval starts in Transaction Console but ends with a Failure status after running for about 1 to 2 minutes.
