Getting Error After Clicking On 'Apply' And 'Ok' Both For Adding Records In Subtab

Symptoms
Having one object (A), added as a subtab for another object (B), then the user will have the option to add/associate records from object A with object B.
For example Agreement (custom object) is added as a subtab for Partner (Standard Object).
When the user tries to add Agreement records under Partner, if will click on 'Apply' and 'OK' both and after a very short time, is getting an error.

"You can't add this record because it already exists"
STEPS

The issue can be reproduced at will with the following steps:
1. Add Agreement records under Partner;
2. Click on 'Apply' and 'OK' both;
3. Getting the error -"You can't add this record because it already exists".
