RIB Service Monitor Shows Injector Service Down With FailedAuthentication (Doc ID 2936850.1)

Goal

In RIB-EXT, RIB Service Monitor tab is showing Injector Service as status down with reason code (exception/error):

FailedAuthentication:Client received SOAP Fault from server: Failed to assert identity with UsernameToken. Please see the server log to find more detail regarding exact cause of the failure.
