Attributes, Removed from OTM Item Template, Are Still Getting Applied on Target Items

Symptoms
Even if an attribute is removed from an Item Template in OTM, that attribute is still getting added on an target item when the same Item Template is applied.
Example and Steps to Replicate:
1. Created one Item Template in OTM with an attribute of Reference Number (i.e. Refnum).
2. If this Item Template is applied on any item now, the Refnum will also be tagged on the target item.
3. Now, removed the Refnum attribute from the Item Template.
4. Trying for applying the same Item Template on another Item - that removed Refnum is added on the Target item.
Note :
1. It is not happening almost all tries but it is inconsistent.
2.Similar observation is there for Remarks and other attributes like Item Template Name as well.
