When attempting to create business license record type, user click on submit origination from design business license application intake form ,
the following error occurs.

ERROR

Your intake form couldn't be processed. Check the information you have entered and try again. Contact the agency if the problem continues.


STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. In OPAL,
2. Create business license record type,
3. Click on design business license application intake form
4.Add contact > view Form > Click on Origination > submit
