Why is the OIC MS Office 365 Outlook Connection 'Provide Consent' not working?

In Chrome browser the 'Provide Consent' button spawns a new page with a prompt that asks for user name and password.
Entering the credentials of the Service Account that is currently logged into OIC results in the same credentials dialog box being displayed again with no indication of authorization status.

Repeated entry of the same credentials eventually locks the Service Account, which is the only indication that the authorization is failing.
