When specific user attempts to update charge account in Edit Requisition Preferences, an error is returned

The issue cannot be reproduced at will. Steps to duplicate:
1. Navigate to
Home > Procurement >Purchase Requisition >More Tasks >Update Requisition Preference

Requestor, Deliver-to Location, Destination Type (Expense) are automatically populated

Enter Nickname, 

Enter Charge Account combination

The system returns an error

ORA-01427: Single-Row Subquery Returns More Than One Row.
