Edit Icon Is Not Displayed Above DV Workbook in OAC (Doc ID 2975624.1)

Symptoms

When opening a workbook, the dark gray top banner does not display where the Edit icon and others are accessible.
If the user clicks the Browser back button, the workbook lands into the Visualize mode and the banner region with icons displays. The user can now edit the workbook.
This behavior occurs for all users and only for this 1 workbook.
