Cannot Add Planned Service Histories

Symptoms
If the field officer (user) changes the activity's start time from the default time, he is not able to add the planned service histories even when they are mandatory/required. The user needs to log out and re-login to the application to be able to add the mandatory/required planned service histories and complete the activity.
