Symptoms
When attempting to open a Reports report either from the Graphic User Interface (GUI) or Smart View, the following error occurs.
ERROR

Failed to execute report due to an unexpected failure from the data source.
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. From Reports tile->Reports Library, open a report either in HTML or PDF.
2. From Smart View add-in, open a Reports report.
