Access Request Creation in Access Governance Immediately goes into Failed State (Doc ID 3004582.1)

ACTUAL BEHAVIOR
---------------
Not able to raise access request in Oracle Access Governance (OAG/AG) when user has multiple AG Roles assigned in Oracle Cloud Infrastructure (OCI). I am not able to raise access request. It fails right away without any proper error message.


EXPECTED BEHAVIOR
-----------------------
It should work even when there are multiple AG roles assigned.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Assigned UserXYZ as both the AG_Administrator Role in OCI and one or more AG Application Roles like AG_User or any of the AG_* Roles.
2. Logon as said above user to the AG UI
3. Raise a request for a Role for self
4. See that it goes directly to a Failed state
