Symptoms
After adding an Action Plan in a case, the plans were not displaying in the "Actions and Plans" section of the Case. The Actions and Plans section is not showing on the Case Management detail page either.
ACTUAL BEHAVIOR

Actions and Plans tasks are not displaying under "Actions and Plans" section in Case Management
EXPECTED BEHAVIOR

Action plan should be displayed under Actions and Plans section of the case
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Add an Action Plan in a case
2. Initiate the action plan
3. The action plan does not show under Actions and Plans section of the case
