When creating an activity using the create_activity button and finishing it, a clone of this activity is created and remains in the list of pending activities.

OFS should not clone the activity in this case

ACTUAL BEHAVIOR:

In some cases (intermittently and no pattern found), a clone activity is created upon finishing an activity

1. Create an activity (Standard action screen label:create_activity)
2. Execute and finish the activity in question.
3. In some cases, a clone activity is created and kept on the pending activities pool of the resource
