Symptoms
While completing a Health Survey, in the Survey Responder Name field, the drop down is displaying both Employee record and Pending Worker record for the same user.
When searching for the same user in Person Management, you can see the user has 2 records - employee (active) and pending worker (inactive).
STEPS TO REPRODUCE

Complete Health Survey > select Survey Responder Name field > duplicates are displayed.
