Symptoms

The following error appears whenever an employee tries to answer the health survey.

Error:
Exception During RestAction

 

Steps:
1. Login as employee
2. Me > Submit a WHS incident for you or someone else
3. Select check box 'Worksite assessment from home' (Health Survey)
4. Click on continue 

Added all privileges/Roles/DSP from role ORA_HNS_ENVIRONMENT_HEALTH_AND_SAFETY_MANAGER_JOB to the custom Employee role but still issue
Even tried with seeded employee role, same issue.
