Symptoms
Once Termination is processed, a role is added to the user.
The 'Send Pending LDAP Requests' process is run after the role is added. After the process is run, a few are not assigned the role.
Checking from per_ldap_requests, the following error message is showing:
Request set to COMPLETE because a TERMINATION request has already been processed for this user.
