ACTUAL BEHAVIOR

We are unable to update the "Instructions and Help Materials" for the existing questionnaire.

EXPECTED BEHAVIOR

Should be able to edit questionnaire

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Navigate to My client groups -> Profiles -> Questionnaires
2. Search for Questionnaire
3 Edit
4. Unable to add help instructions

BUSINESS IMPACT
-----------------------

The issue has the following business impact:

Due to this issue, users cannot add help instructions to existing questionnaire
