Symptoms

Upgraded Oracle Forms 11g(11.1.2.2.0) to 12.2.1.4.0 using Domain Reconfiguration. Started Admin Server and WLS_FORMS managed server.

Running the Oracle Forms servlet shows below error

Changes

 Installed 12.2.1.4 Fusion Middleware and Oracle Forms and Reports. Reconfigured the existing 11.1.2.2.0 domain to upgraded with 12.2.1.4.0 as per below documentation.
