OCI Operations Insights : Newly Enabled Database / Existing Databases Reports "OPSI-DBCS-0005" Error

Symptoms
For the Databases where the OPSI enabled, following error is being reported:
OPSI-DBCS-0005:Cannot read the array length because "" is null
Scenario 1:
Existing OPSI users can hit this issue once they upgrade their Exadata System / DB System Version to 23.1 till 23.5.
From the Ops Insights Administration page, We can see the DB state reported as "Needs attention". Click on more gives the following error,
Error : OPSI-DBCS-0005:Cannot read the array length because "" is null
Another symptom would be missing Ops Insights metrics. This would also be indicated within the "Collection issues" dashboard.
Scenario 2:
Enable the OPSI for a Database.
DB state reported as "Failed". Click on more gives the following error,
Error : OPSI-DBCS-0005:Cannot read the array length because "" is null
