"Limited Document" Action Label is Getting Changed to "Accept Document" and Throwing Error When Executing from OTM/GTM Custom Screenset

Symptoms
OTM / GTM Customer is having an observation on GTM Trade Transaction screen.
They are using custom screenset for this screen. Here, they have an action called "Limited Document". If this gets edited and saved without changing anything- the label is getting updated as "Accept Document".
Now, if the same custom screenset is added as part of any menu, then while executing/triggering this action "Accept Document", it is throwing error.
