Symptoms
ACTUAL BEHAVIOR
---------------
When creating a subscription and saving the page refreshes and the field values are blank.
The subscription does save but the user is unable to see it or re-query the subscription.
EXPECTED BEHAVIOR
-----------------------
When saving the subscription the data should be saved on the page and the subscription number will be shown.
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Navigate to the Create Subscription page
2. Enter data in fields and save
3. The field labels are displayed but the values are blank and the subscription number is not shown in the header.
