SSO Login Failure with No User Returned Error (Doc ID 3053541.1)

Symptoms

After a quarterly update, SSO login fails on all users.

Users see an error "System error. Please re-try your action. If you continue to get this error, please contact the Administrator."

Testing SSO from Security Console results in failure with the following message. (Security Console > Single Sign-On > Single Sign-On Configuration: Identity Provider Details > Diagnostics and Activation > Test Identity Provider)
