11gR2 Grid Infrastructure Installation Fails with INS-40901 or INS-40937 as Vendor Clusterware is Configured but Unhealthy (Doc ID 1056223.1)

Symptoms

11gR2 Grid Infrastructure installation fails with INS-40901 or INS-40937, "Cluste Node Information" window shows empty list and "Add" botton is greyed out;  installation logs located in $ORACLE_BASE/oraInventory/logs/installActions{$TIMESTAMP}.log shows similar entries like following:

INFO: Found Vendor Clusterware. Fetching Cluster Configuration
INFO: Executing [/tmp/OraInstall2010-01-25_03-43-28PM/ext/bin/lsnodes]
 with environment variables {..LD_LIBRARY_PATH=/tmp/OraInstall{$TIMESTAMP}/jdk/jre/lib/sparcv9/server:/tmp/OraInstall{$TIMESTAMP}/jdk/jre/lib/sparcv9:/tmp/OraInstall{$TIMESTAMP}/jdk/jre/../lib/sparcv9..}
INFO: Starting Output Reader Threads for process /tmp/OraInstall2010-01-25_03-43-28PM/ext/bin/lsnodes
INFO: The process /tmp/OraInstall2010-01-25_03-43-28PM/ext/bin/lsnodes exited with code 1


SEVERE: [FATAL] [INS-40901] The cluster node information table is blank.
  CAUSE: The cluster node information table is blank.
  ACTION: Provide valid data in the cluster node information table.
INFO: Advice is ABORT
INFO: Adding ExitStatus INVALID_USER_INPUT to the exit status set

Or:

[FATAL] [INS-40937] Following hostnames are not valid:{0}
  CAUSE: Hostname specified may be an alias.
  ACTION: Ensure that valid hostname is specified.
