ORA-7445 [Qkxrpxformkcc()+197] after Applying Patch 7456731

Symptoms

After applying the patch for bug:7456731 several core dumps under qkxrPXformKcc() in different processes are seen, eg.:
ORA-07445: exception encountered: core dump [qkxrPXformKcc()+197] [SIGSEGV] [unknown code] [0x000000000] [] []

The current SQL statement from the crashes session shows that ORA_ROWSCN is used.

Other possible symptoms are:
ORA-7445 [qkexrFindMatching()+56]
ORA-600 [qkexrFindMatching1]
Call stacks will look similar to:
