Client Connections Fail With TNS-12154 / ORA-12154

Symptoms

Connection to the database, error with ORA-12154: TNS:could not resolve the connect identifier specified.

In this case,TNSping also intermittently failing, reporting:

TNSPING.EXE - Application Error : The instruction at "0x609fc63c" referenced memory at "0x0000007c". The memory could not be "read".
