Clusterware Fails to Start due to CSSD Fails to start with "Duplicate voting file found" (Doc ID 1274309.1)

Symptoms

Using Grid Infrastructure 11gR2, there is some storage change, causing device mapping changed. After correcting the device mapping, rebooting the node, Grid Infrastructure fails to start. ora.cssd fails fails with the following message everytime it tries to start:
