Grid Infrastructure Installation root.sh Failed with "Failed to start CTSS"

Symptoms

New installation of 11.2.0.1 Grid Infrastructure on a 4 node cluster using OUI, root.sh succeeds on nodes 1 and 2, but fails on nodes 3 and 4 with the following messages:
