Symptoms

11gR2 Grid Infrastructure root script (root.sh or rootupgrade.sh) fails with:

Failed to add (property/value):('OLD_OCR_ID/'-1') for checkpoint:ROOTCRS_OLDHOMEINFO.Error code is 256

The directory "/opt/oracle" specified for diagnostic_dest parameter is not writable on nodes "racnode1". Make sure that the directory is writable and retry.

ASM rolling upgrade action failed, see logs for details
Failed to upgrade ASM in a rolling fashion for Oracle clusterware  configuration at /opt/grid/11.2.0.2/crs/install/crsconfig_lib.pm line 12661.
/opt/grid/11.2.0.2/perl/bin/perl -I/opt/grid/11.2.0.2/perl/lib -I/opt/grid/11.2.0.2/crs/install /opt/grid/11.2.0.2/crs/install/rootcrs.pl execution failed
