Symptoms
2 node RAC, 11.2.0.2 Grid Infrastructure with a 10.2.0.4 database, the database is de-registered from OCR, refreshed from a backup, then registered again into OCR. When adding back the service, get error:
srvctl add service -d RACDB -s rac_srv -r RACDB1 -a RACDB2 -P BASIC
PRKP-1029 : Failed to register the service rac_srv.
CRS-0168: Cannot create the backup file.

Changes
Deregister a database from OCR, then register it back again.
