SMON Terminates Database Instance with ORA-15081 With Failgroup Down (Doc ID 1316024.1)

Information in this document applies to any platform.


On RAC set up, you have normal or high redundancy diskgroup and one failgroup failed or switched off.
You also have switched off one node immediately. You see the database instance on the other node dies with the following errors:

ORA-15081: failed to submit an I/O operation to a disk
Tue Apr 12 10:39:23 2011
System state dump requested by (instance=2, osid=28357 (SMON)), summary=[abnormal instance termination].
System State dumped to trace file /<DIR>/rdbms/<DBNAME>/<SID>/trace/<SID>_diag_28302.trc
SMON (ospid: 28357): terminating the instance due to error 15081

On this node, the ASM instace and the diskgroup stay mounted.
