Database Crash With ora-4030 Using ORADISM and With 'oradism creation failed' message.

Symptoms
Solaris OS is being used.
The oradism creation failed for unknown reasons and "0 8 101" errors have been arising for some time. Eventually the database crashed with the errors shown below.
