OneCommand Installer Failing with [FATAL] [INS-40908] Invalid host names in step 13 (Doc ID 1330854.1)

Even with a successful execution of checkip and OneCommand step 0-12 running OneCommand step 13 "GridSwInstall" may fail with the message "[FATAL] [INS-40908] Invalid host names":

CAUSE: One or more host names you provided are invalid, as they do not resolve to a valid IP address.
ACTION: Enter valid host names.
*EXCEPTION DETAILS:*
[INS-30081] ex02db02 could not be resolved using TCP/IP host name lookup. Provide valid name that can be resolvable using TCP/IP host name lookup.
A log of this session is currently saved as: /tmp/OraInstall2011-06-14_02-23-40PM/installActions2011-06-14_02-23-40PM.log. Oracle recommends that if you want to keep this log, you should move it from the temporary location to a more permanent location.

The following list of files is checked and found to be correct:

• checkip output
• grid.rsp: oracle.install.crs.config.clusterNodes
• gridinstall.sh
• onecommand.params: HostArr
• dbm.dat entries

Also: manual and automated ping and look-up test are all successful.
