Symptoms

Attempting to run an rman active duplicate fails:

RMAN-03009: failure of backup command on prmy1 channel at 06/21/2011 11:21:57
ORA-19558: error de-allocating device
ORA-19557: device error, device type: DISK, device name:
ORA-17627: ORA-01041: internal error. hostdef extension doesn't exist
ORA-17627: ORA-01041: internal error. hostdef extension doesn't exist
ORA-03113: end-of-file on communication channel
channel prmy1 disabled, job failed on it will be run on another channel
