ORA-600 [kqlidchg1] ORA-00001: Unique Constraint (Sys.I_plscope_sig_identifier$) Violated (Doc ID 1341014.1)

Symptoms

After upgrading a database from 11.1.0.7 to 11.2.0.2 , you can find the following error when running utlrp script.

ORA-00600: internal error code, arguments: [kqlidchg1], [], [], [], [], [], [], [], [], [], [], []
ORA-00604: error occurred at recursive SQL level 2
ORA-00001: unique constraint (SYS.I_PLSCOPE_SIG_IDENTIFIER$) violated


The failing statement mentioned as the current sql in the trace file is:

begin dbms_utility.validate(:1,:2,:3,:4); end;


The same trace file shows a call stack similar to:

... kqlidchg kqlchg kglfls1 ktcCommitTxn kqlvld kglgob psd_validate pevm_icd_call_common pfrinstr_ICAL pfrrun_no_tool pfrrun plsql_run peicnt ...
