What diagnostics should be gathered to investigate the warning "Last autorefresh for interval nnn ms has not finished..." (Doc ID 1357186.1)

Goal

This warning is generated when an autorefresh operation has stalled:

08:15:54.91 Warn: ORA: 15129: ora-15129-0009-fresher00854: Last autorefresh for interval 30000 ms has not finished after 300000 ms

The ms values depend on the defined autorefresh interval. The warning will be repeated with incrementing ms values. However no other information is generated making it difficult to know where the stall lies. This Note provides a list of diagnostics to gather to try to isolate and pinpoint where the problem might lie.
