Exadata - Create RAC Database Fails With ORA-15204 (Doc ID 1358896.1)

Goal

You are attempting to create a RAC database on Exadata using DBCA.

DBCA fails with the following errors:

ORA-00200: control file could not be created
ORA-00202: control file: '+DATA_GRP'
ORA-17502: ksfdcre:4 Failed to create file +DATA_GRP
ORA-15001: diskgroup "DATA_GRP" does not exist or is not mounted
ORA-15204: database version 11.2.0.0.0 is incompatible with diskgroup DATA_GRP

The errors in the alert log show:

Errors in file /u01/app/oracle/diag/rdbms/dbm/dbm1/trace/dbm1_ora_17556.trc:
ORA-00200: control file could not be created
ORA-00202: control file: '+DATA_GRP'
ORA-17502: ksfdcre:4 Failed to create file +DATA_GRP
ORA-15001: diskgroup "DATA_GRP" does not exist or is not mounted
ORA-15204: database version 11.2.0.0.0 is incompatible with diskgroup DATA_GRP
ORA-1501 signalled during: CREATE DATABASE "DBM"
