Symptoms

You can successfully connect as sysdba but a subsequent startup of the database fails with an ORA-1012:

/home/oracle>sqlplus / as sysdba
SQL*Plus: Release 11.2.0.2.0 Production on Thu Oct 6 11:03:24 2011
Copyright (c) 1982, 2010, Oracle.  All rights reserved.
Connected.
SQL> startup
ORA-01012: not logged on


Environment parameters like NLS_LANG , ORA_NLS33 or ORA_NLS10 have been verified and are set correctly.
 

Changes

Oracle has been forcefully shutdown at OS level or crashed.
