"There is a Problem with Your Enviroment" or Login Page is Blank After New Install or Upgrade of APEX

Information in this document applies to any platform.
After completing a successful install or upgrade of APEX 4.x, when attempting to bring up either the APEX or APEX Admin pages, the login page is not presented / is blank. An error similar to the following may be presented:
Error: 'apex' is undefined
In 5.0 and above, you may see the following error message:
This can occur with a new install and / or upgrade of APEX 4.x. This error can happen with the following configurations:
- Oracle HTTP Server
- Embedded PLSQL Gateway (EPG)
- Oracle REST Data Services / APEX Listener
