1) +FRA or +RECO diskgroup (External redundancy) got corrupted and cannot be mounted again.

2) +FRA or +RECO diskgroup contains the multiplexed redologs, multiplexed control files and multiplexed archivelog location.

3) Therefore database(s) cannot be opened.
