Oracle Database Upgrade Assistant - Version 11.2.0.0 and later
Oracle Application Express (APEX) - Version 4.2.5.00.08 to 4.2.5.00.08 [Release 4]

Symptoms

During upgrade to 11.2 or 12.1 using DBUA  from 10g/11g database  , DBUA automatically install the APEX even if APEX is not been installed in source database
