Symptoms

During RMAN backup, the following errors are generated in the alert.log file:
ORA-07445: exception encountered: core dump [V_vsnprintf()+163] [SIGSEGV] [ADDR:0x7FFEA672A604] [PC:0x2AFD746D94BA] [Address not mapped to object] []
and ORA 7445 [ksfq_aio()+404]

The stack trace in the trace file generated is similar to:

_vsnprintf <- V_snprintf <- dbclientMessage
<- writeToServer <- dbc_put <- bsa_put <- VxBSASendData <- xbsa_SendData
<- int_WriteData <- sbtwrite2 <- skgfwrt <- ksfqsubmit <- ksfvdoaio
<- ksfvrdp <- opirip <- opidrv <- sou2o

The process is waiting for the media manager layer. This can be found by searching the trace file for 'Current Wait Stack'. The process in question is

'waiting for 'Backup: MML write backup piece''.

The RMAN log just shows:

RMAN-00569: =============== ERROR MESSAGE STACK FOLLOWS ===============
RMAN-00571: ===========================================================

RMAN-03009: failure of backup command on ch00 channel at 03/24/2012 04:45:50
ORA-00447: fatal error in background process
