Exadata : Database Instances are not starting after the Grid Infrastructure upgrade to (Doc ID 1446886.1)

After upgrading the grid infrastructure to, database instances fail to come up.

Alert log file shows, startup hangs at the mount stage . The call stack of DSKM indicates, it was trying to communicate
with the clusterware diskmon process.

Installation of Database home using BP15 or higher with Grid Infrastructure home using or lower.

This issue will be hit in the following upgrade circumstances:

* The Grid Infrastructure home is running or earlier, and an Database home is upgraded to BP15 or later.

* A Database home is running BP15 or later, and the Grid Infrastructure home is upgraded from to or earlier
