Add Disk to Diskgroup Fails with ORA-00600 [kfgso::badobj] ( ORA-07445 kfioRqSetPrepare ) (Doc ID 1447862.1)

Symptoms

While adding the disk to a ASM diskgroup ,ASM instance got crashed with ORA-600 [kfgso::badobj]
immediately rebalance process started after updating other related ASM insformation into the diskgroup metadata.
Which ultimately resulted in ORA-07445 [kfioRqSetPrepare()+2020] and crashed the ASM instance.
 
 
SUCCESS: alter diskgroup dgrecover3 add disk '/dev/rhdiskpower48','/dev/rhdiskpower49','/dev/rhdiskpower50','/dev/rhdiskpower59','/dev/rhdiskpower60'

 Apply the patch 11833884 as per the readme to the Grid Home.This patch fix has been included in 11.2.0.3 onwards.
 On lower version,need to apply the patch 11833884 if available for your platform ,if not need to request for one-off patch to oracle support  for your environment.
