ORA-7445: [kglbrk()+11] Followed By ORA-600: [kgldafr-bad-ext] And Instance Crash

Symptoms

The instance alert file shows the following errors followed by an instance crash:

ORA-07445: Exception [type: SIGSEGV, Address not mapped to object] [ADDR:0x80] [PC:0x938518B, kglbrk()+11] [flags: 0x0, count: 1]
...
ORA-00600: internal error code, arguments: [kgldafr-bad-ext], [0x109FD735E0],[], [], [], [], [], [], [], [], [], []
...
LGWR waiting for instance termination
PMON (ospid: 17344): terminating the instance due to error 472


The following errors might also occur:

ORA-07445: [kglbrk()+11]
ORA-00600: [kglbrk-bad-lock]
ORA-00600: [kgldafr-bad-ext]
ORA-00600: [kglpin-bad-lock]
ORA-00600: [kss_get_type: bad control]

Review of the generated tracefiles for the ORA-07445: [kglbrk()+11] error reveals a call stack similar to:
