During to upgrade, fails and reports "USM driver install actions failed"

The log files show that oracleadvm and oracleacfs modules are not loaded.


[root]# /u01/app/11.2.0/grid/

   ORACLE_OWNER= oracle
   ORACLE_HOME=  /u01/app/11.2.0/grid


Entries will be added to the /etc/oratab file as needed by
Database Configuration Assistant when a database is created
Finished running generic part of root script.
Now product-specific root actions will be performed.

Modules are successfully loaded using insmod (manual)

[root ]# lsmod|grep oracle
