This note outlines the sort of initial information that is likely to be requested when logging a Service Request (SR) related to performance problems in the database. Obviously each problem is unique so there may be some differences in specific cases. The Note contains a sample Action Plan to provide  Oracle Support with the initial information required to understand the problem and start working upon it.
Questions from the Action Plan can be proactively addressed by Customers to the best of their knowledge while logging Performance SRs and in doing this they may also assist by providing some self-assessment information that might push towards a resolution for the problem.

The article is aimed at engineers and customers who may want to know the expected level of information and diagnostics for Performance SRs.
