RBAL HIT ERROR ORA-600[KFDVFGETCURRENT_BADDSK],FAILED TO START ASM (Doc ID 1493050.1)

Symptoms

 You have an 11.2 ASM diskgroup for voting file, OCR and spfile. This diskgroup is HIGH redundancy, with 5 disks(this can also happen with NORMAL redundancy diskgroup with 3 disks).
  2 of the disks have failed. When replacing and trying to bring disks online the CSS will not recognize the change 
 and adjust voting disk count to a majority.When disks containing voting file failed,
 user may then encounter the rebalance (RBAL)  ORA-600[KFDVFGETCURRENT_BADDSK] 
 and the ASM instance will crashed.
 
 

Changes

 Disks went offline and are being brought back online.
