Symptoms

As described in "Known Issues" note for Exadata software image 11.2.3.2.0 (<note 1468877.1>), it is possible the upgrade to Exadata Database Server software image 11.2.3.2.0 fails in adding the hot-spare to the hardware RAID5 (HW RAID5). So far, the following Exadata images are confirmed to see this issue upgrading to 11.2.3.2.0: 11.2.2.4.2, 11.2.3.1.0. However, it expected more image versions are impacted.
