Symptoms

SELECT UTL_INADDR.GET_HOST_ADDRESS host_address FROM dual RETURNS ORA-1422

query generates the following error for user

SQL> SELECT UTL_INADDR.GET_HOST_NAME('xxx.xxx.xxx.xxx') host_name from dual;

SELECT UTL_INADDR.GET_HOST_NAME('xxx.xxx.xxx.xxx') host_name from dual
  *
ERROR at line 1:
ORA-01422: exact fetch returns more than requested number of rows
ORA-06512: at "SYS.UTL_INADDR", line 4
ORA-06512: at "SYS.UTL_INADDR", line 35
ORA-06512: at line 1
