Symptoms

Database corruption caused by lost write (IO) due to Hitachi storage micro code defect where one side of the mirror gets out of synch.

Configuration:

Virtual Storage Platform (VSP) by Hitachi Data Systems. 

The Oracle Database may produce different errors or wrong results pointing to lost IO or reading a stale/old version of a block:
